2/14/2024 EnchantaVaughn Approved QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Kaila verbalized to QMHPA that her day was going well but she is still depressed about a lot of things going on in her life. Kaila did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Kailaâs current thoughts, feelings and emotions. QMHPA discussed with Kaila about being able to recognize resourcefulness in in dealing with depression/anxiety. QMHPA assisted Kaila with clarifying and refine the coping skill she uses frequently. QMHPA engaged Kaila in a discussion surrounding her being able to implement coping skills to life stressors that cause her to become depressed. QMHPA engaged her in a discussion to determine the coping skills she has utilized in the past to assist her with coping skills. QMHPA informed Kaila that the purpose of this activity was to assist Kaila with applying past coping skills she used to overcome situations in the past and apply them to past situations that may cause depression. QMHPA discussed with Kaila about applying positive self-talk by increasing the frequency of positive thinking and talking about herself, the world and the future. QMHPA explained to Kaila about taking a deep breath and replace negative thinking with a realistic positive view of life. QMHPA encouraged Kaila to journal her feelings daily and identifying a positive way to express her feelings. Kaila appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. During the skills building activity, Kaila said she thinks highly of herself and feels positive about herself. Kaila became outspoken and was able to identify positive things about herself.
